Elfen Lied mostra uma evolução da humanidade, os Diclonius. Os Diclonius se parecem muito com humanos normais, porém, possuem algumas diferenças: Uma glândula pineal de tamanho exacerbado, um par de pequenos chifres em suas cabeças, olhos e cabelos avermelhados/rosados, e conseguem sentir a presença de outros de sua espécie. Como resultado da mutação na glândula pineal, competem à telecinese; desenvolvem estruturas de natureza desconhecida, de ação física similar à de braços, denominados vectors. A força que tais estruturas possuem o poder de meramente reduzir uma cabeça humana à frangalhos, e parar tiros. Por causa dessas características especiais, os Diclonius sofrem preconceito; tanto por medo de suas potencialidades quanto por interesse da espécie humana. Uma relação de extermínio e uso se estabelece; os humanos desejam exterminar os diclonius, mas antes, aproveitam-se deles para pesquisas, e os diclonius desejam eliminar os humanos por vingança pelos maus tratos e sofrimentos causados pelos humanos.

O diclonius (ser humano com dois chifres) é um mutante criado por seres humanos. Tem uma habilidade especial que poderia ser descrita como um sexto sentido. Os mutantes tem poder suficiente para eliminar os humanos do mundo. Os seres humanos que estão receosos de seus poderes, e por isso os diclonius foram isolados nos laboratórios de pesquisa para serem estudados. Uma das meninas diclonius, chamada Lucy, conseguiu sair do confinamento, matou os protetores e escapou do laboratório. Ela cai no oceano após terem disparado tiros nela, acaba indo parar em uma praia. Ela acabou perdendo a memória com a queda, e na praia encontrou Kota e Yuka, que lhe deram o nome de Nyu e começaram a viver junto a ela.

A história do anime se passa em um "Japão feudal futurístico e pós-apocalíptico", onde, aquele que conseguir a bandana simbólica de "Número Um"(o homem mais forte do mundo), irá comandar o mundo, como um "deus". Alguém se torna o Número Um matando o antigo Número Um e tomando sua bandana. Entretanto, somente aquele que tiver a bandana de "Número Dois" tem direito de desafiar o Número Um. Por sua vez somente o número três pode desafiar o número dois, e assim por diante até o número sete, onde todos podem desafiá-lo. Mas isso muda quando o novo número um tem todas as outras bandanas, exceto a de número dois (sob domínio de Afro), então todos podem desafiar o número dois e somente o número dois pode desafiar o número um deixando a missão de Afro quase impossível. Quando alguém completar as sete bandanas, ninguém poderia desafiá-lo, mas nada impede de que alguém mais forte o mate e roube as bandanas.

Quando criança, Afro teve seu pai (que era o Número Um) assassinado bem em sua frente pelo pistoleiro "Justice" que passou a ser o novo Número Um. Agora Adulto, Afro Samurai, é o atual Número Dois, e viaja pelo mundo em busca de vingança,passando por cima de qualquer empecilho pelo atual Número Um que matou seu pai

Na história, o "Death Note" é um caderno todo em cor preta e que tem seu nome escrito na parte frontal dele. Esse caderno tem a capacidade de matar qualquer pessoa apenas se os nomes destas forem escritos nele enquanto o portador visualizar mentalmente o rosto de quem quer assassinar. Normalmente os nomes das pessoas que são escritos no caderno morrem de um ataque cardíaco depois de quarenta segundos, a menos que se especifique a causa da morte.^[2]

Os shinigamis são os portadores originais dos Death Notes e graças aos seus olhos podem matar as pessoas facilmente, já que lhes permitem ver o nome verdadeiro e o tempo que lhes resta de vida.^[3] Os shinigamis, por serem deuses espirituais, não podem ser vistos por pessoas normais ao menos que elas tenham tocado em seu Death Note.^[14] Quando Ryuk deixou o seu Death Note na Terra, ele escreveu as instruções sobre o seu uso em inglês, já que ela é uma língua universal; assim, Light Yagami aprende a usá-lo instantaneamente ao encontrá-lo, mas no decorrer da série, aparecem outras regras.^[15]

No mangá, cada capítulo contém uma página especial com as regras do Death Note,^[2] enquanto que no anime, em cada pausa — geralmente duas por episódio — se informa uma regra de como usá-lo.^[16] No momento em que um humano portador de um Death Note renuncia a este, ou o caderno é destruído, ou essa pessoa perde todas as lembranças relacionadas ao caderno automaticamente, mas se a pessoa o tocá-lo novamente, as lembranças retornarão
